- HN is the HCPCS Level II modifier for bachelors degree level.
- With some payers, yes. BCBS Oklahoma's modifier policy assigns HN when the rendering provider is an RBT, a BCaBA, or a bachelor's-level clinician.
- 24 per 15 minutes).
What HN, HM and HO mean
CMS maintains the HCPCS Level II codes and modifiers. The education-level modifiers that ABA payers borrow are:
| Modifier | HCPCS descriptor |
|---|---|
| HM | Less than bachelor degree level |
| HN | Bachelors degree level |
| HO | Masters degree level |
| HP | Doctoral level |
The descriptors describe education level, not ABA credentials. Each payer decides how to map RBT, BCaBA and BCBA onto them, which is why the same 97153 line can need HN for one payer and no modifier for another.
A commercial payer example
Blue Cross and Blue Shield of Oklahoma's Modifier Reference Policy (CPCP023) gives an ABA-specific mapping:
- HM: the rendering provider has less than a bachelor's degree and no Registered Behavior Technician (RBT) certification.
- HN: the rendering provider is an RBT, a Board Certified Assistant Behavior Analyst (BCaBA), or a clinician with a bachelor's degree.
- HO: the rendering provider is a BCBA, a BCBA-D, or a clinician with a master's degree.
Under a policy like that, an RBT delivering 97153 bills 97153 HN. Other payers publish their own tables, and some ignore these modifiers. Always use the payer's current policy, not a habit carried over from another plan.

Florida Medicaid uses HN differently
Florida's Behavior Analysis Fee Schedule (January 1, 2025) lists 97153 with no credential modifier, payable at $12.26 per 15 minutes when provided by an RBT, a BCaBA, or a Lead Analyst. HN appears on other lines, where it marks a BCaBA:
- 97155 HN: protocol modification by a BCaBA, $15.37 per 15 minutes (97155 by a Lead Analyst is $19.17).
- 97156 HN: family training by a BCaBA, $15.24 per 15 minutes (Lead Analyst $19.05).
Florida's schedule also uses TS (behavior reassessment on 97151), GT (97156 by telemedicine, up to 2 hours per week), XP (concurrent supervision, not reimbursed), and UN, UP, UQ, UR and US for two through six clients in a group on 97154 and 97158. These are fee-for-service maximums. SMMC plans, which pay most Florida Medicaid ABA claims since February 1, 2025, may require their own modifiers.
Denials a wrong credential modifier causes
| Code | X12 definition |
|---|---|
| CARC 4 | The procedure code is inconsistent with the modifier used. |
| CARC 185 | The rendering provider is not eligible to perform the service billed. |
| CARC 16 + RARC N822 | Missing procedure modifier(s). |
| CARC 16 + RARC N823 | Incomplete/Invalid procedure modifier(s). |
| CARC 284 | Authorization number may be valid but does not apply to the billed services. |
The last one surprises people. When an authorization is written for a code-and-modifier pair, billing the code with a different modifier can fall outside the auth.
Getting it right every time
- Keep a per-payer table: code, required modifier by credential, and the link and date of the policy you took it from.
- Drive the modifier from the rendering practitioner's credential on the session, not from the scheduler's default.
- Update the table when a technician becomes a BCaBA. Florida's coverage policy notes that a change of practitioner status, such as an RBT becoming a BCaBA, is not grounds for a reassessment, but it can change the modifier and rate on your claims.
- Check the auth: does it list the code with the modifier you are about to bill?
A pre-claim check that reads the rendering practitioner's credential from the note and compares the code and modifier with the payer table catches these before they become CARC 4 denials.
Watch out. Payer contracts and plan manuals can add requirements beyond the rules summarized here.
