Where the rules live

Florida covers behavior analysis (BA) under the Florida Medicaid Behavior Analysis Services Coverage Policy, incorporated by reference in Rule 59G-4.125, F.A.C., and adopted effective February 10, 2025. Codes, modifiers and maximum fees are in the Behavior Analysis Fee Schedule, incorporated in Rule 59G-4.002. General documentation rules are in Rule 59G-1.054. Beginning February 1, 2025, BA services are reimbursed by Statewide Medicaid Managed Care (SMMC) plans: claims for services on or after that date go to the recipient's plan, and recipients not enrolled in a plan stay in fee-for-service. Plans must follow the coverage policy and may not impose more stringent coverage limits than it does (section 1.2), but their billing manuals add plan-specific rules.

Units: the 8-minute rule

Section 8.2 says providers may only claim services performed for 8 minutes or more when computing billable 15-minute units. For services over 15 minutes, divide total minutes by 15. A remainder of 8 minutes or more counts as one unit, and a remainder under 8 is not billable. So 52 minutes is 3 units (7 minutes left over), and 53 minutes is 4 units. Use the units calculator to check a session.

Limits and exclusions worth knowing

Authorization and claim type

For fee-for-service, obtain authorization from the QIO before starting services and at least every 180 days after, and request a new authorization when a new assessment is clinically required (7.2). School-based requests need the IEP or the documented alternatives the policy lists. Claims are professional (837P/CMS-1500), with the most current code, modifier and diagnosis to the highest level of specificity, including co-occurring disorders that affect medical necessity (8.2–8.4.1).

Whatever software you use, the pattern is the same: the claim is built from the session note, so most of these problems can be caught before the claim is submitted.

Watch out. Payer contracts and plan manuals can add requirements beyond the rules summarized here.
